| The Haunted Nephilim functions similarly to the [[Infinite Forest|Haunted Forest]] in which players must complete "sectors" of the Nephilim by clearing out the room of high priority Nightmares to progress to the next room. In between sectors, invincible [[Headless One]]s wielding [[Hive Axe]]s will charge at players and it is advised to ignore them and move away from their line of sight.

| After completing seven sectors, players will be teleported to a boss room in which they must face a generic Nightmare boss. They have no unique mechanics but modifiers can make it more difficult to fight them. After defeating seven bosses, they will encounter a rotating Nightmare Final Boss of great notoriety. Upon reaching third of its health, they will become immune and summon in two Headless Ones in which players must kill them and launch their Pumpkin Bombs at the boss to break its shield. When reaching the final third of its health, the boss will become immune and players must complete the process again. After defeating the final boss, players will be awarded the final chest.
